Addressing Treatment Interfering Behaviors and Treatment Refusal

Program Description:

This two-part presentation explains why some people behave in ways that are incompatible with recovery, and focuses on working with both individuals and families. Part One provides approaches to help individual clients benefit from therapy and Part 2 focuses on strategies to support the well-being of the whole family. Recovery Avoidance often presents a barrier to successful treatment outcomes.

Sometimes when clients present for therapy, Treatment Interfering Behaviors (TIBs) get in the way of their progress. Therapists often work through ambivalence with clients but may be frustrated when the client is not able to achieve their goals due to missing sessions, struggling to follow-through with tasks, or difficulty accurately reporting their difficulties. The Readiness Approach directly deals with these TIBs so that the client can more effectively participate in treatment. In this presentation, attendees learn to focus treatment directly on the TIBs:

  • Identifying the TIB to target first
  • Developing and implementing interventions to address the factors that drive the TIB
  • Returning to the treatment of the presenting problem after meeting the operational outcome goals that indicate readiness.

When individuals struggling with their mental health do not seek the help they need, their family members often struggle to understand why, especially when the disorder negatively impacts their lives. Familiess are often neglected and frustrated by patient-focused treatment models when their loved one is not participating. They may wrongfully conclude the treatment refuser is willfully choosing to be impaired, which can lead them to react in ways that are counterproductive and result in substantial family conflict. Attendees of this presentation learn about Recovery Avoidance and how to respond to it constructively using the Family Well-Being Approach. They also learn how to apply five steps so families leave the session with useful clinical tools. The 5 steps of the Family Wellbeing Approach (FWBA) are:

  1. Prepare for the crisis
  2. Clarify the problem & set realistic goals
  3. Increase activity you value
  4. Decrease family distress
  5. Promote recovery-compatible behavior in the family member with recovery avoidance.

Clinical Objectives:

At the close of this presentation, attendees should be able to:

  • Articulate the definition of recovery avoidance.

  • Describe the 4 factors that can contribute to recovery avoidance.

  • Describe the Steps to address TIBs

  • Identify 2 common family responses that can make recovery avoidance worse.

  • Describe the 5 Steps of the Family Wellbeing Approach (FWBA).

  • 2 CE Contact Hours: For Psychologists in all states, Social Workers in New York, Virginia, and Washington, DC, and CHES professionals. Many licensing boards accept CE from APA approved sponsors as well as for trainings approved by boards in other jurisdictions. Presented by: Melanie VanDyke, PhD

Melanie VanDyke earned her Ph.D. in clinical psychology from the University of Nebraska, where she helped establish the Anxiety Disorders Clinic. Dr. VanDyke completed her Postdoctoral Fellowship in Intensive Treatment for Individuals with OCD and Related Disorders at the Saint Louis Behavioral Medicine Institute, mentored by Alec Pollard. She has extensive experience in evidence-based treatments for anxiety and related disorders.

Dr. VanDyke and her colleagues at SLBMI published research on Recovery Avoidance: improving treatment outcomes for individuals with treatment refractory symptoms and addressing family well-being when an individual refuses treatment. These projects culminated in the publication of the self-help book: When a Loved One Won’t Seek Mental Health Treatment: How to Promote Recovery and Reclaim your Family’s Well-Being. Dr. VanDyke trains and consults with clinicians to help these families with the Family Well-Being Approach.
