RX Refill Request
To request a prescription refill online, you will need to login to your Onpatient Portal. You should have received a login already, but if not, you can contact us using one of the phone numbers below.
To login, please go to www.onpatient.com.
Please include ALL of the following information in your request. The request cannot be fulfilled if you do not respond to each of these questions:
Physician's name
Name of medication
Medication strength
Medication instructions
Preferred Pharmacy: Name, Address, and Phone
Date of next appointment - or when appointment is due if not yet scheduled
Date you will be out of medication
You can copy-paste this list of questions into the portal and complete them from there.
www.onpatient.com is a secure portal, however you should be aware that a member of our patient care team will be processing your request. Therefore, do not leave confidential information for your clinician. If you need to contact your clinician, or have any questions, please call one of the phone numbers listed below.
You can expect to have your refill within 48 (business) hours. Please reach out by phone if there is an urgency to your request.
